Since the sister only ONE brother, that brother MUST be that man himself. So that boy must be his son. The boy can only be the man's nephew if he is the son of his brother which is NOT possible because that man is his sister's only brother.

only brother implies that she has only one brother and that is the man who points the boy so, the boy is the son of the man
