f ( g ( x ) ) = 4 c x + 8 + c = 12 x + d f\left( g(x)\right) = 4cx+8+c=12x+d

As this an identity, equating the coefficient of x x both side, we finally get

4 c = 12 c = 3 4c=12 \Rightarrow c=3 and 8 + c = d d = 11 8+c=d \Rightarrow d=\boxed{11}
