Let what the shopkeeper receives be + + and gives out as - . Then the transactions are as follows .

Transaction Rs. Rs. Received from Lady + 0 Gives to Neighbour 0 Received from Neighbour + 1000 Gives to Lady 200 goods 800 Return to Neighbour 1000 Total + 1000 2000 Loss 1000 \begin{array} {lrrl} \underline{\text{Transaction}} & \underline{\text{Rs.}} & \underline{\text{Rs.}} & \\ \text{Received from Lady} & +0 & & \\ \text{Gives to Neighbour} & & -0 & \\ \text{Received from Neighbour} & +1000 & & \\ \text{Gives to Lady} & & -200 & \text{ goods} \\ & & -800 & \\ \text{Return to Neighbour} & & -1000 & \\ \hline \text{Total} & +1000 & -2000 & \\ \hline \text{Loss} & & \boxed{1000} & \end{array}

I think the correct answer here is Rs.1200 instead of Rs.1000.

While Rs.1000 is the loss incurred due to the fake note, the lady also bought goods worth Rs.200 with that same note. The shopkeeper thus lost an additional Rs.200 in revenue.
