How do you make a 'choose all the correct answers' problem?

I have an idea for a problem where you are presented with multiple options and choose all that apply. I have seen a few problems like that but I don't see the option when creating a problem.

How do you make it have square boxes for choices?

yeah that option seems to have disappeared

I've never seen it as an option. I don't need it right away as I found a better way of doing my most recent problem.
