Before you can get the average, first get the summation of the units times the grades(Σwx) then, also the summation of the units.(Σw). Then you can now divide the summation of units and grades to the summation of the units.

In the last two rows, what does the entry 4.00/3.00 mean?

Sir, still, it's a grade. Equivalent to 3.00, In our school, if you got a grade of 4.00, you will have two chances depending on the removal exam, either you get 4.00/3.00 - passed or 4.00/5.00 - failed. I'm sorry to confused you Sir.

It's better if you state the meaning of 4.00/3.00 on the question. First time, I assumed the 4.00/3.00 is 4.00 divided by 3.00, so my answer is 2.16.
